BSA Health Form Update
The BSA Annual Health and Medical Form #680-001
Parts A and B are Required for all participants at most Unit, District or Council Event lasting less than 72 Hours Day Camp is considered a less than 72 hour event
BSA Annual Health and Medical form #680-001 Parts A & B
Parts A, B and C are required for all Participants at any Unit, District or Council Event lasting more than 72 Hours or when the event takes place more than 30 minutes from a road accessible to emergency vehicles
